01｜老花為什麼點一滴眼藥水，就能看清楚一點？ 眼睛看近，靠的是水晶體變厚。年輕時水晶體軟，睫狀肌一拉就變形；40 歲以後水晶體慢慢變硬，拉不太動了。 所以老花是對焦的彈性沒了，跟近視、遠視的度數是兩回事。眼藥水沒辦法讓水晶體變回柔軟。 它們用的是另一招：把瞳孔縮小。 道理跟相機縮光圈一樣。光圈縮小，景深變深，前後都比較清楚。LENZ 說它的藥能把瞳孔縮到 2 毫米以下，靠的就是這種「針孔效應」。 五款藥都在縮瞳孔，差在怎麼縮。 瞳孔周圍有兩條肌肉在拔河。一圈環狀的括約肌負責收緊，一片放射狀的擴大肌負責撐開。 Vuity 和 Qlosi（都是 pilocarpine，毛果芸香鹼）、Vizz（aceclidine）都是去拉緊括約肌。pilocarpine 會順手拉到睫狀肌，有人因此頭痛、看遠變糊，國外也有罕見視網膜裂孔的案例回報。Vizz 主打幾乎只動瞳孔，不碰睫狀肌。 Ryzumvi 走反方向。它擋住擴大肌上的 α1 受體，擴大肌撐不開，瞳孔就自己縮回來。開發商 Opus Genetics 說，這樣不碰睫狀肌，看近變好、看遠不受影響。 1 月剛核准的 Yuvezzi 則兩招合用：carbachol 拉括約肌，brimonidine 壓住擴大肌。 ![老花眼藥水的作用機制：多數藥拉緊瞳孔括約肌，Ryzumvi 放鬆擴大肌](https://drugnews.com.tw/assets/articles/ryzumvi-presbyopia-eye-drops-viatris-partner/figure-01.png) *圖1｜五款藥都靠縮小瞳孔，Ryzumvi 是唯一只放鬆擴大肌的一款* ## 02｜睡前點一滴，隔天早上會差多少？ Ryzumvi 最特別的是用法：傍晚或睡前點。 三期試驗 VEGA-3 收了 545 人，每天傍晚點一次。第 8 天、點藥後 12 小時，27.2% 的人看近多讀出 3 行以上，看遠也沒變差；安慰劑組是 11.5%。第一天點完 1 小時，是 20.6% 對 6.1%。 大約每四個人，有一個人有明顯感覺。 拿 Vizz 對照。三期 CLARITY 2 點藥後 30 分鐘和 3 小時，都有 71% 的人看近多 3 行，10 小時還有 40%。兩個試驗量法不同，Vizz 量單眼、Ryzumvi 量雙眼，數字只能當參考，落差還是很明顯。 Ryzumvi 贏在頭痛少。VEGA-3 頭痛只有 2.6%；Vizz 三期扣掉安慰劑後的頭痛率是 7.6%。代價是眼睛紅：仿單寫結膜充血 21%、點藥處刺激 16%。 **Ryzumvi 解決的是早上醒來找不到眼鏡，離「一整天不用眼鏡」還很遠。** ## 05｜台灣還買不到，誰最適合點？ 台灣目前沒有核准任何老花專用的眼藥水。東森新聞 10 月 12 日報導，第一款最快 2027 年有機會引進，但沒有說是哪一款、哪家公司。 台灣有一個美國少見的難題：高度近視的人多。三軍總醫院眼科醫師呂大文接受自由健康網訪問時說，高度近視的人點了縮瞳藥，可能覺得近視又多了 100 到 150 度，最適合的反而是輕微遠視約 150 度的人。 他也提到，台灣做過雷射近視手術的人超過 100 萬，這群人可能最有興趣。 眼科醫師鐘珮禎說得更直接：「老花眼藥水只能暫時改善近距離視力，並不能治療老花本身。」 所以就算 2027 年真的引進，台灣的客群也會是一小塊：做過雷射、又不想在餐桌上掏出老花眼鏡的人。 對大多數看手機要伸直手臂的人來說，Ryzumvi 帶來的消息只有一半是好的：眼藥水確實有效，最划算的答案還放在眼鏡行。 本文為產業與公司分析，不構成投資或醫療建議。 ##
