Price cuts eat revenue. Novo Nordisk proved it this year.

Its first-half report, published on August 4, said U.S. sales of injectable Wegovy fell 22% at constant exchange rates in the second quarter, mainly because of lower realised prices.

Eli Lilly's third quarter, reported on October 29, says the opposite. Lilly's realised prices fell 15% worldwide. The volume it shipped rose 53%.

Revenue reached $24.40 billion, up 39% from $17.60 billion a year earlier.

Lilly then lifted its full-year revenue guidance from $85–87 billion to $91–92 billion. It was the third time this year the company raised its outlook.

Same price war, two outcomes. One company is guiding to a shrinking year. The other is accelerating.

The difference is who built the capacity to catch the new patients that cheaper drugs bring in.

**When Lilly issues its 2027 guidance early next year, the midpoint of its revenue range will be at least $100 billion.**

## 01 | Prices fell 15%. How did revenue rise 39%?

Start with Lilly's own numbers. In the United States, realised prices fell 7% and volume rose 41%, taking U.S. revenue to $15.1 billion, up 34%.

Outside the United States the swing was bigger. Prices dropped 29% and volume jumped 76%. Revenue there came to $9.3 billion, up 47%.

The flagship, Mounjaro, booked $10.72 billion in the quarter, up 65%. A year earlier the same quarter brought in $6.52 billion.

One detail matters more than the headline. Mounjaro's sales outside the U.S. were $5.7 billion, up 93%, which is now more than its $5.0 billion inside the U.S.

The first thing lower prices unlocked was countries that could not afford the drug before.

Zepbound, the same molecule (tirzepatide) approved for weight loss, added $5.35 billion, up 49%.

Together the two brands brought in about $16.1 billion, roughly two-thirds of Lilly's quarterly revenue.

*Outside the U.S., prices fell 29% while volume rose 76%, the single biggest growth engine of the quarter.*

Conventional wisdom says price cuts crush margins. Lilly's reported gross margin was 83.9% of revenue in the third quarter, one percentage point higher than a year earlier.

Why does that matter? Because the usual fear about drug price cuts is that they hand money from shareholders to payers and leave nothing behind.

Lilly's quarter shows a different mechanism. A lower price did not just transfer value; it changed who could buy.

The volume line, not the price line, ended up deciding the quarter.

Scale also explains the margin. When volume grows this fast, each dose carries less fixed factory cost, which offsets the lower price.

On the earnings call Lilly said it had broken ground on two more U.S. sites: $6.5 billion in Houston and $5 billion in Richmond, Virginia, for $11.5 billion in total.

## 02 | Where the new patients came from

If a price cut only lets existing patients pay less, revenue shrinks. Most of Lilly's extra volume this quarter came from people who were not in the market before.

The clearest example is Medicare's GLP-1 bridge programme, which started on July 1. Lilly said more than half a million people have filled a Lilly prescription through it.

Ilya Yuffa, who runs Lilly's U.S. business, added two figures on the call.

Seven in ten new bridge patients started on a Lilly obesity medicine, and about 80% of bridge patients were new to this class of drugs.

That is a new pool, not a discount for the old one.

Total U.S. prescriptions for incretin drugs used in obesity grew 82%, and Lilly holds about six in ten of them.

Chief executive David Ricks summed it up: "Lilly had another strong quarter, exceeding our prior expectations and allowing us to raise our full-year outlook."

*A low-price entry point brings in first-time patients; manufacturing capacity decides how many can be served.*

This is also why the bridge numbers matter more than they look.

Medicare patients are older, often carry diabetes or heart disease, and tend to stay on chronic drugs once they start.

A patient who begins on Mounjaro or Zepbound through a government programme is likely to be a patient for years, which turns a one-time price concession into a long revenue stream.

Lilly's market share among new patients is the number to watch.

Seven in ten new bridge patients chose Lilly, a higher rate than its roughly six-in-ten share of all U.S. obesity prescriptions.

New patients are choosing Lilly at a faster rate than the installed base, which means its share should keep rising as the market grows.

The same logic showed up in South Korea in October.

In our piece on [Hanmi's half-price obesity shot](https://drugnews.com.tw/articles/2026-10-20-korea-glp1-price-war-hanmi-eppe-novo-en.html), the first brand to feel the pain was also Novo's Wegovy.

## 03 | Novo is winning the pill race and losing the overall ledger

To be fair, Novo Nordisk holds strong cards. Its Wegovy pill launched in January.

It sold DKK 3.22 billion in the second quarter, passed five million cumulative U.S. prescriptions, and held about 90% of the U.S. oral obesity market by mid-July.

Lilly's pill, Foundayo (orforglipron), was approved by the FDA on April 1, roughly four months later.

It generated $184 million in the third quarter and $281 million so far this year.

It is catching up fast.

Lilly said about four in ten patients starting an oral obesity medicine in the third quarter chose Foundayo, and the number of new Foundayo patients doubled from the second quarter.

Both companies charge the same $149 a month for cash-pay starter doses, part of a drug-pricing deal with the White House.

On efficacy, the Wegovy pill delivered about 14% average weight loss in trials and Foundayo about 11% at its top dose. The two have never been tested head to head.

The problem is size. Novo's pill brought in DKK 5.47 billion in the first half, not enough to make up for price erosion on its injectables in America.

In August Novo set its 2026 adjusted sales growth outlook at 0% to minus 6%, and that was already an upgrade from minus 4% to minus 12% in May.

*Lilly raised revenue guidance to $91–92 billion; Novo expects 2026 sales to be flat to down 6%.*

*Novo leads in pills, but injectable and international volume tilt the overall ledger toward Lilly.*

The international gap is just as clear. Lilly said it now holds about 58% of the incretin market outside the U.S.

Foundayo is already on sale in the UAE, the UK, Saudi Arabia and Mexico, with reviews under way in more than 40 other countries.

Novo's international business is not weak. Its second-quarter sales outside the U.S. grew 10%. But price cuts are holding back its American business, where sales grew only 4%.

The bear case deserves a full hearing.

Lilly's chief financial officer, Lucas Montarce, said about $2 billion of 2026 revenue came from one-time items, including milestone payments and changes to rebate estimates, and will not repeat next year.

He also said net realised price will remain a headwind in 2027, though he expects it to be "more than offset by volume growth."

Novo, for its part, will cut the U.S. list price of Wegovy by about 50% and Ozempic by about 35% from January 1, 2027. The price war is about to get harsher.

That cut matters for Lilly too. In the U.S., list prices are the starting point for rebates and discounts paid to insurers and pharmacy benefit managers.

When the market leader in one molecule class slashes list prices, rivals face pressure to follow, and the net price for the whole category resets lower.

Lilly has not announced an equivalent 2027 list price cut, but its CFO's warning about a price headwind suggests the company expects to give up more ground.

Earnings per share also look weaker than revenue.

Lilly booked $4.18 billion of acquired in-process R&D charges in the third quarter, mainly for its AtaiBeckley and Curevo acquisitions.

As a result, it trimmed its full-year non-GAAP EPS guidance from $35.50–36.50 to $35.00–36.00, even as revenue guidance went up.

## 04 | What it takes to reach $100 billion

Do the arithmetic. The midpoint of Lilly's 2026 guidance is $91.5 billion. Strip out the $2 billion of one-time items and the base is about $89.5 billion.

Reaching $100 billion in 2027 requires roughly 12% growth.

Assume prices fall another 15% next year. Volume would need to grow only about 32% to get there. In the third quarter, volume grew 53%.

Annualise the third quarter alone and Lilly is already running at about $97.6 billion.

In other words, most of the distance to $100 billion has already been covered by the current run rate; the question is whether the price cuts in January wipe out more than the new volume adds.

Is the assumption conservative? U.S. volume grew 41% and international volume 76% last quarter, and the Medicare bridge has run for only one quarter.

As long as volume growth stays above about 30%, the price cuts cannot catch up.

The next products are lined up too.

Lilly said Phase 3 trials of eloralintide plus tirzepatide will start in the fourth quarter.

That is in line with our earlier view in [Lilly's two-drug obesity bet](https://drugnews.com.tw/articles/2026-10-14-eloratzp-weight-loss-tolerability-persistence-en.html).

Retatrutide beat Ozempic 2 mg in a head-to-head Phase 3 diabetes trial, with a bigger A1C reduction and more than double the weight loss.

When new drugs take over, Lilly does not need to defend old prices.

There is also a case that $100 billion is too cautious. Foundayo is still early, with a U.S. type 2 diabetes decision expected in the fourth quarter.

Kisunla, Lilly's Alzheimer's drug, grew 175% to $193 million. Jaypirca, Ebglyss and Omvoh each grew between 59% and 76%.

None of these is large yet, but together they show Lilly is no longer a single-franchise story.

The real constraint is supply.

A price cut only turns into revenue if there are enough doses to sell, and injectable pens and oral tablets each need their own production lines, fill-finish capacity and quality checks.

The $11.5 billion in new U.S. plants is the company's answer, and the volume numbers this quarter suggest the investment has started to pay off.

The last time we looked at [Lilly's obesity franchise in May](https://drugnews.com.tw/articles/2026-05-08-lilly-tirzepatide-glp1-growth-en.html), the story was high prices and a small, wealthy patient base.

Six months later the business model has flipped: low prices, sold to many more people.

A 15% price cut looks like a giveaway. For Lilly, it is the entry fee for the next wave of patients.
